*Most insurance plans consider nutrition counseling to be a preventative service, however, some plans consider it a medical service. Here’s how your visit might be covered:

*Covered at 100% with no patient responsibility

*Payment subject to specialist copay, deductible and/or coinsurance

Out of Network Reimbursement

*Depending on your specific plan, we can provide you with a superbill (itemized receipt for healthcare services) and you may be able to file a claim for reimbursement with your insurance company.

Self-Pay Clients

*Payment will be due at the end of each visit and your credit/debit card on file will be charged.

$140 for your first session (60-75 minutes)

$120 for your follow up sessions (60 minutes or $35 per 15-minutes prorated amount)

Insurance Coverage

Aetna: in-network provider (EPO Connect plans require a referral from a patient’s primary care physician)

Anthem / Blue Cross Blue Shield: in-network provider
Cigna: in-network provider (except for EPO Connect plans)
Humana: not currently accepted
Medicaid: not currently accepted
Medicare: in-network (patient MUST have a diagnosis of diabetes or chronic kidney disease)
Optima: in-network provider
Oscar: out-of-network, but a single case agreement may be obtained
Tricare: not currently accepted
United: in-network provider

Out of Pocket Rates

Initial visit (1 hour minimum): $140 for 1 hour; $35 for each additional 15 minutes

Follow-up rates: $60 for 30 minutes; $30 for each additional 15 minutes

Follow-up package: five 30-minute visits for $275
